Artist Application

4th Annual Lot 6 Art Fair
Saturday, June 14th 2014 10am to 5pm
Lot #6, Irvine Ave & 4th St (Downtown Bemidji)

This event is open to all artists interested in selling and displaying their artwork and fine crafts. ALL work must be original in design and made by you/your business. Absolutely NO imports, kits, commercial jewelry, molded ceramics or manufactured items will be allowed. All applications will be submitted to a jury committee. Acceptance will be decided by originality, craftsmanship, quality and consistency. A $10 non-refundable jury fee must be submitted with your application.

Booth spaces are 10x10 (no electric), you must supply all your own tents, table, etc… All artwork must be done by you/your business, if you are wishing to share a booth w/an artist, both Artists must apply and be selected. Limited double booths (10x20) are available on a first come first serve basis. We are also selecting a limited amount of artists to get a free upgrade to a double booth space if demonstrations can be shown. Please describe in detail how you would demonstrate your craft in your booth, to be considered for a double booth space.

Deadlines & what you will need to submit checklist:

•  Completed form including artist statement w/images emailed or on CD. Incomplete applications will not be accepted, please take care to complete the form in full.

•  $20 Early Bird application due/postmarked by Feb 20th (10x10 booth)
• +$15 Early Bird 10x20 booth ($35 total by Feb 20th)

• $30 booth Application due/postmarked by April 17th (10x10 booth)
• +$25 10x20 booth ($55 total by April 17th)

• $10 non-refundable jury fee check (waived for returning artists w/same medium)

•  Proof of insurance and/or waiver (attached below)

• Completed ST-19 form (http://taxes.state.mn.us/forms/Documents/st19.pdf)
• (All exhibitors are responsible for collecting/reporting MN state sales tax)

*Notifications of acceptance will be emailed no later than May 1st (if you do not have an email address, please enclose a self-addressed stamped envelope w/your application.
*Promotional material will be available to participating artists soon after notifications are sent.

Event will take place Saturday, June 14th RAIN or SHINE. Limited Friday, June 13th setup evening times for early set up/check in to be announced. Saturday, June 14th setup/check in begins at 8am. Event begins promptly at 10am, please be setup at that time, late setups will not be allowed. Tear down begins at 5pm, please no early tear downs.

Submission of form indicates commitment to participate, artists wishing to cancel will be refunded the booth fee only if submitted in writing before May 17th.
No refunds will be awarded after May 17th.

in the Lot 6 Art Fair. I do hereby expressly release and hold harmless the Lot 6 Art Fair, its committee & volunteers, RailRiverFolkSchool and the City of Bemidji from any liability of injury, loss or damage to personal property associated with participation in this event. I acknowledge that I understand the waiver described in this document. Waiver is made to the maximum extent permissible under applicable law. I acknowledge that I have signed this document under my own free will.
